Er...a little help please?

I couldn't seem to find any tutorials saying how to actually make a ball-jointed doll, but I've looked at a bunch of pictures(I don't have a real doll) and I think I know how it works for the most part. There are still some important things I don't get:

How does the head attach to the neck?

I think you put string through all the limbs starting at the neck. Is that right? What happens to the string when it gets to the hands and feet? And at the neck?
